About the role:
The Care Coordinator assumes the role of delivering advanced clinical support, leadership, and coordination of clinical care tailored to residents/clients with complex needs. In this role, you will offer mentorship and guidance to a care team comprising Personal Care Workers (PCWs) and Registered Nurses within our facility.
Duties:
- Supervise and organise the completion of baseline and required focus assessments for individual residents/clients, recording findings within a Positive Wellbeing Model.
- Ensure the development and timely review of care plans based on individual residents/clients' assessment findings, adhering to organisational timelines.
- Coordinate the accurate collection of clinical/care data for the timely submission of the Aged Care Funding Instrument, monitor documentation accuracy, and oversee the provision of care by external service providers.
- Current AHPRA Registration as a Registered Nurse and hold minimum 2 years of Aged Care experience
- Experience managing and leading teams within the Aged Care sector
- Strong knowledge of Aged Care Accreditation standards and ACFI/AN-ACC Funding modules
