An exciting opportunity is currently available for an experienced Clinical Nurse (Case Manager) to join the Integrated Care Rehabilitation Services Team at Hervey Bay and Maryborough Hospitals within the Wide Bay Hospital and Health Service. Your opportunity:
- To provide expert clinical and patient-centred care within Integrated Care Rehabilitation Services by supporting the facilitation and provision of quality and cost effective nursing services which are consistent with contemporary standards and evidence-informed practice.
- The Transition Care Program is an exciting jointly funded initiative of the Commonwealth Department of Health and Ageing and Queensland Health, offering time-limited and goal oriented flexible rehabilitation care packages to older persons who have had a recent acute or sub-acute hospital admission and live within the Wide Bay Health Service region. It is aimed at older clients, but age alone is not an eligibility criteria.
- The position of Clinical Nurse (Case Manager) will have responsibility for the assessing, care planning, provision of nursing care, coordinating and monitoring of allocated clients within the Transition Care Program. It also involves case management leadership and partnership working with Non-Government Organisations and external service providers who provide care and therapy to Transition Care clients.
