About the roleWe are seeking motivated and enthusiastic clinicians employed with Grampians Health to become part of the Emergency Department Care Coordination (EDCC) team for 8 hours per fortnight permanent part time with the option to backfill leave.
The EDCC team facilitate timely access to an integrated system of community-based services.
This includes:
- facilitating safe and timely discharge to the community
- providing person-centred care that encourages and supports consumer and carer participation
- providing a primary access point within the hospital ED for community based services
- promoting continuity of care within and across hospital and community
The Grampians Health Emergency Department services the Grampians region for all urgent medical needs. Consisting of 25 beds, with 12 short stay units and 3 resuscitation bays.
We offer a mix of Emergency Department skill sets with triage, resuscitation cubicles, acute cubicles, minor injury - fast tracks, and short-stay unit. Open 24-hours, there is a variety of rotating shifts.

Skills and experience
- Have extensive experience in acute and community health and working in interdisciplinary teams.
- Experience in assessment of people with complex needs, discharge planning, case conference and care planning within a multi-disciplinary team.
- Have a passion for and experience in providing high quality patient centred care
- Have excellent interpersonal communication skills
- Will enjoy working in a fast-paced dynamic environment
- Ability to collaborate with the ED team
- Be able to work casual EDCC shifts when available
- Current AHPRA Registration

This position is paid in accordance with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2020 - 2024. The classification is Registered Nurse Grade 2 (YP2 - YP9) which ranges from $34.17 - $48.26 per hour.
All appointments are subject to a satisfactory police records check, immunisation clearance and Working with Children check and/or NDIS Worker Screening Check.
All employees must be willing and able to wear Personal Protective Equipment (PPE) when required, which at a minimum is a surgical mask. Additional PPE requirements are role-dependent.
