Moorditj Koort Aboriginal Corporation is seeking three (2) dedicated professional to join our team and play a pivotal role working collaboratively with clients, general practices, AMSs, and other service providers to assist with the care coordination of eligible clients with chronic disease.
Our team assist Aboriginal and/or Torres Strait Islander people to understand their chronic health condition and how to manage and to follow their care plan, which may include support for chronic disease self-management and assistance with care plan compliance.

Moorditj Koort Aboriginal Corporation (MKAC) is an Aboriginal community controlled, not-for-profit organisation supporting the health and wellness of Aboriginal and Torres Strait Islander peoples living in Western Australia. The Vision is to contribute to healthy and flourishing Aboriginal communities across WA by providing access to a holistic one-stop-shop for health and wellbeing support.
Responsibilities and Duties:- Arranging the required services outlined in the client’s care plan, in close consultation with their home practice.
- Work collaboratively and connect your clients to a range of health professionals, including specialists, GPs, nurses and allied health professionals.
- Transferring and updating the client’s medical records in the MKAC client data system.
- Undertaking activities to increase the awareness and understanding of relevant Closing the Gap initiatives.
- Ensuring there are arrangements in place for the client to get to appointments.
- Assists identify gaps in specialist and allied health services that compliment general practice chronic disease management and support.
- Transport clients to and from health appointments.
- Valid WA Driver’s license
- Aboriginal Health Practitioner cert IV or equivalent health qualification e.g., nursing
- Australian Health Practitioner Regulation Agency (AHPRA) registration
- National Police Clearance no more than 3 months old
- Current Working with Children’s Check
- Current First Aid Certificate or willing to obtain.
- Covid-19 Vaccination certificate (MyGov, Medicare, GP document)
