Vacancy in our Care Finder team: Care Finder
Care Finder is a Commonwealth Government funded program commissioned by Primary Health Tasmania. The program aims to connect and engage older people who experience difficulty accessing aged care services. Care Finders provide one-on-one support for older Tasmanians to interact with My Aged Care and access aged care services and other supports in the community. Care Finders are based in each of Tasmania’s three major regions.
COTA Tasmania has been contracted as one of four providers of Care Finder services in Tasmania.
We are recruiting for a Care Finder in our small Burnie office. The Care Finder will:
- support people through registration, screening and assessment for aged care services;
- support people to identify, connect with and access services that are appropriate to their individual needs, preferences and circumstances;
- perform high level check-in with clients to see if services are still in place and meeting needs; and
- provide follow up support if needs change or services have lapsed.
If you have knowledge and experience of the aged care system; enjoy supporting others; can explain complex systems in a clear way; and want to work in a small (but organised and fun!) team, this could be your new role for 2024.
