Position Overview: We are seeking a dedicated and experienced Care Manager who is a Registered Nurse (RN) to join our growing team. The Care Manager will play a crucial role in coordinating and overseeing the care provided to our clients, ensuring their needs are met with the highest standards of quality and compassion.
Key Responsibilities:
- Assess the health needs and care requirements of clients through comprehensive evaluations.
- Develop, implement, and regularly review personalized care plans.
- Supervise and support our team of caregivers, providing training and guidance as needed.
- Conduct regular home visits to monitor client care and satisfaction.
- Liaise with clients’ families, healthcare professionals, and other stakeholders to ensure seamless care coordination.
- Maintain accurate and up-to-date client records and documentation.
- Respond promptly to any changes in clients' health status or care needs.
- Ensure compliance with all healthcare regulations and company policies.
Qualifications:
- Must be a Registered Nurse (RN) with a current, valid license.
- Minimum of 2 years of experience in a similar role, preferably in home care or a related field.
- Strong clinical skills and the ability to perform thorough assessments.
- Excellent communication and interpersonal skills.
- Compassionate, patient, and dedicated to providing high-quality care.
- Ability to work independently and as part of a team.
- Proficient in using electronic health records (EHR) and other relevant technology.
