Your new role
We are currently seeking a flexible and enthusiastic Carefinder Case Manager to join our dynamic team covering Metro Melbourne, Regional VIC and Tasmania on a Full-Time basis. This position will run from the 11th of March 2024 to the 7th of March 2025.We are seeking candidates with excellent communication skills who can build rapport with those who are socially isolated within the community. Care Finders provide assertive outreach to assist older people who face barriers to accessing aged care and are vulnerable, marginalised, and disadvantaged.
Your responsibilities and duties will include:
- Assessing potential clients, taking into account their history, circumstances and preferences
- Supporting clients to navigate My Aged Care and other services systems including NDIS to ensure they are receiving the services they want, need and are entitled to.
- Encouraging and supporting clients to make decisions regarding their use of services and to manage their own health and welfare needs
- Enhance the social contact of isolated clients by providing social support or linking and referring them into social and recreational activities

We are looking for candidates with the following:
- A qualification in Social Work, Social Sciences, Welfare or equivalent
- Outreach skills including the ability to locate and establish rapport with both socially isolated people and people with challenging behaviours.
- Experience in working effectively and sensitively with a broad range of clients, including those with high support needs and challenging behaviours
- An understanding of the Housing and Homelessness Service sector My Aged Care, NDIS and other relevant service systems.
- A current drivers licence, and reliable vehicle
- Proven commitment to excellence, accountability and innovation in best practice service delivery in the fields of homelessness and housing support.
- Well-developed communication skills and the ability to engage with people from varying backgrounds.
- First Aid Certificate

Successful applicants will be required to undergo a NDIS Worker Screening Check and Police Records check. Refusal to provide your information to both of these parties will prevent Wintringham from being able to continue with your application.
Applicants who have lived in a country other than Australia for a period longer than 12 months in the last 10 years may be required to undergo an International Police Check.
Successful candidates will be required to comply with the Wintringham Health and Immunisation policy including having three doses of a COVID-19 vaccine before commencing employment and must provide evidence.
