Position Title: Case Manager – Home Care Packages
Employment Type: Ongoing part-time 48 to 64 hours per fortnight

Role Description
Heathcote Health is looking for a Case Manager to support our Home Care Package program. Our Case Managers play a critical role in the provision of comprehensive and coordinated case management and support to assist people to live independently in their own homes for as long as they can. Working with our clients you will tailor packages of care to assist them to live at home safely and advocate and liaise with other service providers on their behalf as well as conduct initial and annual home visit assessments of the clients and build trusting relationships with them and their families.
About you
What you bring
- Tertiary qualification and experience in the community/aged/disability sector
- Proficient computer skills with the ability to learn and adapt to new systems and databases
- Experience in working with clients with cognitive impairment
- Understanding of wellness, reablement and client directed care
- A desire to join a team committed to the care of others in a mission based organisation
Experience in case management for Home Care Packages is preferred, but not essential.
