Summary:
The Case Manager will be responsible for managing and coordinating the care of clients, ensuring that they receive the appropriate services to meet their needs. The ideal candidate will have excellent communication skills, be able to work independently and as part of a team, and have a strong commitment to providing the highest standard of care to our clients.
Our Launceston Case Management team are based in a brand-new state of the art building and have their very own clinical team (RN & EN) who are on hand for advice and support. Laptop and mobile phone is supplied as well as access to fleet vehicles. The team services Launceston and surrounds as well as the Northeast and East Coast of Tasmania.
Duties:
- Conduct assessments of clients' needs and develop individualised care plans
- Coordinate and monitor the delivery of services and resources to clients
- Maintain accurate and up-to-date client records and package budget
- Collaborate with other professionals, such as healthcare providers and social workers, to ensure comprehensive care for clients
- Monitor and evaluate the effectiveness of care plans and make adjustments as needed, working collaboratively with our very own clinical team.
- Provide education and support to clients and their families
- Participate in case conferences and team meetings
- Knowledge of the Aged Care Quality Standards, Home Care Package guidelines, local community resources and services
About you:
We are looking for and organised and enthusiastic individual who wants to make a difference and holds the following qualifications and skills.
- Cert IV or Diploma in Community Services/Case Management (desirable)
- Minimum of 2 years of experience in case management of Home Care Packages (desirable)
- Police check (less than 12 months)
- Current WWVP with NDIS endorsement
- Drivers license
