The role:The Assessment and Case Management Service deliver quality care that improves the health and wellbeing of people in the Tasmanian Community. As
Case Manager -
Transition Care Program you'll be responsible for;
- Providing case management services to older persons identified as medically stable ready for discharge from the Acute Care Sector and who would benefit from time limited, goal orientated, therapy focused restorative care, within the Southern Region.
- Complying with the Department of Health and Ageing Transition Care Program Guidelines, Legislation and Reporting requirements.
- Working with older persons to complete their restorative process and optimise their functional capacity through case management within an interdisciplinary Community Transition Care Program team.

- Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time and part time vacancies.
Salary: $79,529 - $91,517 per annum. Our Employer 11% superannuation contribution is on top of this amount.You'll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.Salary range is in accordance with the Public Sector Unions Wages Agreement 2022.

The following checks are to be conducted: * Conviction checks in the following areas:
- crimes of violence
- sex related offences
- serious drug offences
- crimes involving dishonesty
- serious traffic offences
- Identification check
- Disciplinary action in previous employment check.
