We are currently seeking a bilingual Case Manager who has qualification in nursing with two years or more of work experience in nursing. This role will be suitable for a person who is fluent in both Vietnamese and English who is willing to work as a team for a full-time position Monday to Friday 9:00 AM – 5:00 PM based in our head office in Braybrook, Victoria.

Key Responsibilities
• Supporting participants with complex health needs, including performing complex health care procedures e.g., care/management, mental health support, catheter care, restrictive practices, seizure management.
• Training and managing support workers.
• Daily coordination of service management, including scheduling appointments and managing in-home services.
• Rostering a suitable roster for the Participants ongoing care.
• Assessing and listening to participant's needs, wants, and goals accordioning to care plans.
• Provide clinical knowledge and expertise to support individuals.
• Conduct health assessments to participants receiving our services.
• Communicating with relevant stakeholders (Support Coordinators, Occupational Therapists, Psychotherapist)
• Following the direction and expectations of the Team Leader and Manager.
Qualifications and Skills:
• AHPRA Registered/Enrolled Nurse with current registration status is essential.
• Minimum 2 years’ experience in nursing.
• Speaks fluent Vietnamese and English.
• Local, lives in the western suburbs – close by to the head office in Braybrook.
• Strong organisational and time management skill, with the ability to multitask and priorities.
• NDISWC Clearance and Police Check (or willingness to obtain).
• Own registered vehicle and current driver's license.
• Effective verbal and written communication skills to liaise with internal teams and external stakeholders.
