The Opportunity
The Case Support Worker provides culturally sensitive assistance to clients and carers, supporting them in various tasks such as transportation, appointment attendance, and engagement in educational, therapeutic, cultural, or leisure activities outlined in the client's Case plan and designated Targeted Care Package (TCP) funding. The TCP program is designed to facilitate the transition of children out of residential care or help them stay out whenever possible. These tailored packages include allocated support hours aligned with specific goals and needs, which the Support Workers actively pursue. In collaboration with the Key Worker (Case Manager) and Team Leader, who conceptualized the package and are working towards overarching TCP goals, the Case Support Worker plays a vital role in achieving positive outcomes.
The Support Case Worker is responsible for:
- Collaborating with Key Workers (Case Managers) and the Team Leader to strategically plan and organize support activities for clients.
- Executing support activities for clients and/or carers, including transportation, attending appointments, and engaging in educational, therapeutic, cultural, or leisure activities outlined in the client's case plan, with funding allocated through their TCP.
- Undertaking additional support tasks with clients and carers in consultation with Key Workers and the Team Leader.
- Ensuring precise and timely preparation of case notes, meticulously documenting all interactions and activities with clients and carers.
- Actively participating in team meetings or client care team meetings, as determined by the Team Leader.
- Engaging in supervision sessions with the line Manager for continuous professional growth and guidance.
- Complying with the philosophy, policies, work practices, and protocols of VACCA while carrying out duties.
- Committing to ongoing professional development and training as required to enhance skills and knowledge.
Requirements:
- You must have and continue to hold a full Victorian Driver’s Licence and a current employment Working with Children Check card.
