JOB PURPOSE
The primary objectives of the Support Worker are:
To support people in their day-to-day living, ensuring they have opportunities to develop and maintain a valued lifestyle as described within their individual plan.
To provide services to individuals which are developed specifically for their needs and within our “Person Cantered” approach of delivery and reflected within the Adelphi Living vision and mission statement.
To be aware of and sensitive to the needs of people who are from an Aboriginal or Torres Strait Islander background and assist them with their integration and ongoing support with Adelphi Living’s services
The Support Worker is also responsible for ensuring all reporting and administrative requirements are met and services are in-line with Adelphi Living Policy and Procedures and the Disability Services Act 1993.
Primary Support Tasks
Assist people with a disability to undertake activities and meet the goals of their support plan
Provide supports in a courteous, professional, and timely manner
Ensure a safe environment and that participants are free from any form of abuse and harassment.
Support and actively encourage the person’s participation in decision making about their own program.
Support a person with their medication or other medical and adhere to Adelphi Living’s policies and procedures and instructions from medical practitioners.
Assist with all areas of personal care including feeding, bathing, toileting, and grooming.
Assist with participant’s mobility including transfers/hoisting etc.
Physical support and assistance to participate in activities.
Provide support external to the home such as transport for shopping, outing, appointments,
Meal preparations with expectations for quality and wholesomeness
Provide compassionate and effective supports without employing unsanctioned restrictive practices.
Provide support in alignment with the Participant’s Behavior Support Plan.
Identify and report any restrictive practices.
Primary Administration Tasks
Complete daily :
Progress notes and communication books.
Shift reports, medication forms and charts, vehicle logs and any other required documentation.
Food and household item stock checks
Cleaning and maintenance schedules

Other Certification required
- Current First Aid and CPR
- NDIS worker orientation and induction modules certificates.
- Medication and manual handling
- Certificate III in Individual Support
- Current drivers licence
Similar industry experience and/or qualifications in or working towards Certificate III or IV in Disability/Mental Health would be advantageous.
