Are you a dedicated nursing professional with a passion for enhancing care for individuals with advanced dementia? 

We are looking for an experienced Clinical Nurse Consultant to take on a pivotal role in our Care Coordination team, focusing on the development and implementation of an innovative Model of Care for end-of-life (EOL) support. This model will enhance the patient experience through strategic partnerships both within our Local Health District (LHD) and with external organisations.

This role will empower you to enhance the coordination and delivery of end-of-life (EOL) care, emphasizing the provision of integrated, seamless services. You will foster effective collaboration between specialist palliative and dementia care teams, ensuring a patient and family-centered approach to care. 

About You

  • You have current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA) with at least 5 years fulltime equivalent in the speciality field of Dementia and/or Palliative Care and hold a current Australian Drivers Licence.
  • You have high level clinical and non-clinical communication skills that can adjust to the varying demands across the role.
  • You have the ability to apply knowledge in the area of dementia and/or end of life care to practical clinical situations and in the development and delivery of education for a broad range of stakeholders
  • You have demonstrated information technology skills with the ability to collect, report and analyse patient activity with a focus on quality and safety and development of innovative nursing practice.

Where You'll be Working

This position is based at Long Jetty Healthcare Facility and may require travel to other locations across the Central Coast.
