A rewarding opportunity at our Avondale Heights Residential Aged Care Home for an experienced Clinical Care Coordinator. This is a temporary full-time position Monday to Friday 0700-1500 covering parental leave until the end of June 2025.
The role will be responsible for care services including care planning, review and assessments.
What you will bring:
- Dedication to resident care and support
- Excellent clinical knowledge regarding medical conditions, palliative care, and care of people living with dementia including managing challenging behaviours
- High level interpersonal and written skills
- Previous experience as a Nurse Unit Manager / Clinical Manager
- Demonstrated experience in managing staff, including performance management
- Ability to effectively communicate with residents, resident representatives and other health professionals
- A good understanding of the aged care reform agenda, including AN-ACC funding and royal commission recommendations related to Clinical Care
- Current AHPRA Registered Nurse DIV 1 registration with no conditions
