About the Role:

An exciting opportunity is presented to work within the Northern Adelaide Local Health Network as a Clinical Care Coordinator within the Northern Community Mental Health Division.

This position will offer you professional growth opportunities within a supportive and inclusive culture. In this key position you will demonstrate your experience and previous success working as a Registered Mental Health Nurse to ensure high quality care is delivered to our consumers.

AHP3 - The Clinical Coordinator is responsible for the coordination of clinical business and clinical processes of the Community Mental Health Team. Operating as a part of the leadership team, the Clinical Coordinator is an experienced Mental Health clinician who will support clinical service delivery.

Employees in this role accept accountability for the outcomes of clinical practice and for addressing inconsistencies between practice and policy and for developing team performance and a positive work culture in the interest of consumer outcomes.

The incumbent will be required to participate and contribute to the strategic and operational management of the sector as directed by the Sector Manager.

Various practice models may be used to enact this role, including but not limited to:

  • Ensuring that appropriate clinical support processes are in place.
  • Providing direct expert care for an individual or group of consumers and carers.
  • Providing clinical leadership in partnership with the Team Manager and Lead Consultant to a
    multi-disciplinary integrated Community Mental team.

RN3A - The Clinical Coordinator (CC) is accountable for their own practice standards, activities delegated to other Nursing Staff and the guidance and development of less experienced staff.

While fulfilling the role of CC, Level 3, Nurses at this level, provide clinical nursing expertise for specified individual consumers and / or groups and / or consumer populations. The CC uses their clinical knowledge and experience to provide the clinical co-ordination of patient / client care delivery in a defined ward / unit value stream or program within a Health Unit / Community Service.

The CC is accountable for the delivery of high-quality Mental Health Nursing Care which is consistent with the Department of Health quality framework.

In this role, accept accountability for the outcomes of clinical practices and for addressing inconsistencies between practice and policy and for developing team performance and a positive work culture in the interest of consumer outcomes.

The Practice model adopted by Mental Health Service to enact this role in the Adult Integrated Community Mental Health Team, including but not limited to undertaking a combination of primarily providing direct expert nursing care for an individual or group of consumers and providing clinical leadership to nurses.

The Practice model adopted by Mental Health Service for CC to enact this role, including but not limited to undertaking:

  • Primarily providing Clinical Coordination in the team.
  • Provide direct expert nursing care for an individual or group of consumers

The Clinical Coordinator in the Integrated Community Mental Health Team is expected to;

  • Provide expert level nursing care to allocated consumers.
  • Manage clinical business processes for the team.
  • Provide all relevant information regarding new referrals, transfers, planned discharges and overnight contacts.
  • Provide information to the Team Manager of current activity and demand.
  • Ensure accurate record of caseload and workflow on the journey board.
  • Delegate work in collaboration with Team Manager / Psychiatrist Lead Consultant.
  • Coordinate and ensure appropriate handover between shifts.
  • Lead and support clinical reviews.
  • Delegate referrals throughout the shift.
  • Be aware of staff clinical skills and capacity.
  • Support Clinical Roles.
  • Support team flow.
  • Provide clinical and Managerial support to the team, in collaboration with the Team Manager and Psychiatrist Lead Consultant.
  • With the rest of the leadership team, share responsibility for developing, supporting, addressing, and maintaining team culture, appropriate team behaviours and implementation of team values.

To be eligible for this position, you must be:

  • Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate, enrolled in an approved Mental Health course or holds a qualification in Mental Health practice.
  • Have an Appropriate degree or equivalent qualification which entitles registration with the Psychology Board of Australia OR
  • Appropriate degree or equivalent qualification which gives eligibility for full membership with the Australian Association of Social Workers OR
  • Appropriate degree or equivalent qualification which entitles registration as an Occupational Therapist.
