AddressA dedicated Clinical Care Coordinator is required for a medium-sized aged care home in the ACT area. As CCC you will be responsible for managing clinical practices within the facility ensuring person-centered care and excellence in Clinical Care are consistently achieved.
You will partner with the Facility Manager and be part of the Executive Management team. You will lead, develop & mentor your care team whilst building strong working relationships with internal & external key stakeholders.
