Job description

As a Care Manager, you will:

  • Manage client service delivery across the Short-Term Restorative Care (STRC) Programme.
  • Develop care plans aligned with the client’s needs and goals in conjunction with the Aged Care Assessment Team (ACAT) assessment.
  • Undertake marketing and collaborate with external allied health providers, medical professionals, and specialist care providers.
  • Co-ordinate multidisciplinary teams to deliver a best care approach.
  • Utilise your business development skills to drive the growth and utilisation of the Short-Term Restorative (STRC) Programme.
  • Ensure compliance with all relevant legislative and industry standards.
  • Deliver excellent client care for elderly Australians.

We are looking for someone with:

  • AHPRA registered in allied health discipline or RN registration considered
  • Experience in aged care, with knowledge of the Short-Term Restorative (STRC) Care Programme
  • Frontline management skills
  • Current First Aid and CPR certificates
  • A current ACT drivers’ licence, vehicle registration, and access to your own car
  • National Police Clearance or willingness to obtain (myHomecare will organise via WorkPro at no cost)
  • The right to work in Australia
