About the role

My client is seeking an experienced home care Registered Nurse / Case Manager to join their team in regional Western Australia. This is full time opportunity with flexibility in working arrangements between Geraldton and Jurien Bay or home.

What you need

Key responsibilities include:

  • Complete reporting and documentation in line with relevant legislation, regulatory requirements, professional standards, policies and procedures to support home care clients.
  • Provide supervision and leadership to a team of healthcare professionals
  • Deliver specialised nursing care within a Case Management framework
  • Monitor clinical care plans
  • Work closely with Area Managers providing reporting, budgeting and feedback around staff needs and program development.

To be successful in the role you will have:

  • Bachelor of Nursing with current AHPRA registration
  • Demonstrated experience in a Registered Nurse position within aged care
  • Experience with Home Care packages and Case Management
  • Ability to build strong working relationships within a multi-disciplinary clinical team
  • Strong written and verbal communication skills
  • Current National Police Clearance and NDIS clearance (or willing to obtain)
  • Valid flu vaccination and Covid vaccination history
  • Full working rights in Australia
  • Driving Licence and own vehicle
