As a Clinical Care Manager, you will:

  • Deliver excellent client care for elderly Australians, located in the suburbs of Perth.
  • Develop, monitor, and review care plans in consultation with clients, their families and other healthcare professionals.
  • Maintain a caseload of around 60 clients, ranging from low care to high care needs.
  • Use your clinical experience, customer service, and aged care management skills to make a mark on the community. 
  • Monitor the budget of care packages and ensure clients understand service fees.
  • Engage with and learn about multiple Government funding streams including Home Care Packages, Short-term Restorative Care and Commonwealth Home Support Programs. 
  • Engage with new clients to the service to create client-centered goals and genuine, lasting client relationships.
  • Ensure compliance with all relevant legislative and industry standards.
  • Proactively engage in continuous improvement across practice, communicating with the appropriate stakeholders.

We are looking for someone with:

  • Hold a Health-related qualification such as an unrestricted current APHRA.
  • Experience in community care such as aged, with knowledge of Home Care Packages, Commonwealth Home Support Program and Aged Care Quality Standards is desired.
  • Current First Aid and CPR.
  • The right to work in Australia.
  • A current driver's licence, registration and access to your own car with comprehensive car insurance
  • National Police Clearance or willingness to obtain (myHomecare will organise via WorkPro at no cost)
