An exciting opportunity to join the New England Regional Assessment Service with Aspire4Life as aClinical Home Support Assessor. This role will see you support older Australians to continue living independently, by considering supports such as assistive technology, social support services, and community care. This is a community-based role allowing you the opportunity to travel throughout the Aged Care Planning Region to work collaboratively with clients in their homes under the Commonwealth Home Support Program (My Aged Care).
This is a full-time permanent role working Monday to Friday (no weekend or shift work). This position offers the opportunity to participate in innovative projects that promote independence throughout the aged care journey.
We offer a comprehensive 2 weeks of full-time training and orientation held both virtually and face-to-face in our Coolangatta office (all expenses covered).

A typical week as an assessor with Aspire4Life:
- Working independently within your region, utilising your assessment experience to conduct holistic style assessments with a client centred approach focusing on wellness and reablement
- Capturing individual client information and working collaboratively to develop their support plan and unique goals
- Develop strategies to connect people with appropriate services and supports in their chosen community
About you:
- Experience in assessment and goal orientated support planning in aged, community, health or social services
- Confident communicator with a positive approach when collaborating with clients, families, staff, and other stakeholders
- Local knowledge and the ability to network with stakeholders within your region
- Advanced IT skills with previous experience working across multiple platforms (Microsoft Office Suite)
- Previous experience working with KPI's to ensure high quality client outcomes
- Ability to manage your time effectively in an autonomous working environment
What to bring?
- Tertiary qualified Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker with AASW Registration
- Unrestricted Australian driver's licence
