A reputable medium capacity Community Aged Care Centre located in Rockhampton is looking for a dedicated Full Time Clinical Lead to provide support to the Nursing team and deliver a high level of quality care to the residents with a focus on person-centred care.

About the role:
As the Clinical Lead you will support the care and service team to ensure the residents are receiving excellent care. You will:
- Carry out comprehensive nursing assessments for individuals and groups, and formulate care plans that provide optimal quality of life for the residents.
- Coordinate and monitor the planning, implementation and evaluation of service outcomes, liaising with the broader team on care needs.
- You will be reporting to the Care Manager & Facility Manager.
- Provide leadership and support to team members, assisting with orientation of new members, identifying learning needs and providing training and development.
- Participate in the identification and implementation of continuous improvement initiatives.
- The organisation is open to suitable candidates ready to move from an RN role up with the right level of experience.
Requirements:
- Registered Nurse with current AHPRA registration
- Experience with Aged Care electronic care systems is regarded but not compulsory
- Solid understanding of the Aged Care Accreditation standards and AN-ACC processes
- Experience working with older people and their families or representatives
- Must have Australian working rights
