Aspire4Life are thrilled to be hiring ClinicalMulti Program Assessors to join our virtual assessment team, based in our Coolangatta office on the Gold Coast.
This role will see you undertake assessments across the Veterans Home Care (VHC) and Regional Assessment Service (RAS) programs, using a wellness approach to identify needs and recommendations for supports with the key objective of supporting clients to continue living independently.
Our business is experiencing a period of internal growth, therefore we have multiple permanent full time opportunities on offer.
The Role:
- Undertake phone assessments under the VHC program, working towards weekly and monthly targets
- Complete Support Plan Reviews and initiate linking support activities under the RAS program
- In consultation with the client, utilise the recommendations created throughout the assessment to develop an individualised Service or Support Plan
About you:
- Experience delivering assessments or service coordination in Health or Aged Care
- High level computer literacy, including proficiency with MS Office Programs
- Well-developed communication skills and the ability to work independently
- Unrestricted Australian Driver’s Licence
- Tertiary qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist with current AHPRA Registration or Social Worker with eligibility for AASW membership
