What you'll be doing
This position provides a key nursing leadership role in the advanced and complex Care Coordination for at risk patients in conjunction with the multidisciplinary team at Blacktown Cancer and Haematology Centre and across the Sydney West Cancer Network as appropriate.The CNC’s role is to ensure seamless continuum of care between the hospital, the Sydney West Cancer Network and the community. In addition, have an approved Post graduate nursing qualification in specialty, or such other qualification or experience deemed appropriate by organisation

• Demonstrated ability to provide leadership that facilitates the ongoing development of evidenced based clinical practice
• Demonstrated ability to organise and prioritise work.
• Demonstrated effective written and oral communication with negotiation and conflict management skills
• Demonstrated experience working as a member of a multidisciplinary team
• Demonstrated effective interpersonal skills showing initiative and leadership to junior members of the team
