The Clinical Nurse in the GEM at Home will visit patients in their home or equivalent to provide care to older people with complex medical, functional and/or psychosocial needs in lieu of an extended or avoidable hospital admission.
About the Role:
As the Clinical Nurse, your duties may include:
Providing proficient, person centred, clinical nursing care and individual case management to patients
Monitoring patient care plans to ensure appropriate care outcomes are achieved on a daily basis
Oversee the provision of nursing care within a team.
- Participating in clinical teaching, overseeing learning experience, and goal setting for students, new staff and staff with less experience
To be successful in this role, you will need:
- Registered or eligible for registration as a Nurse by the Nursing and Midwifery Board of Australia and hold a current practicing certificate
- At least 3 years, full time equivalent, post registration experience.
- Experience in the management of wound management
- Drivers license required.
7 day rotational roster, shifts between 0800 and 2030 and care is delivered in patient's own homes.
