The position would suit an experienced Clinical Nurse in the aged care industry. This is a unique opportunity to utilize your passion & experience.
Essential Requirements:
- Current Registration as a General Nurse with the Nursing and Midwifery Board of Australia.
- Qualifications and minimum 5 years’ experience as a Registered Nurse. Experience in planning, implementing, and evaluating innovative, evidence based clinical systems and practices.
- Ability to plan and deliver a diverse range of workplan activities concurrently to a high standard.
- Excellent written and verbal communication skills including an ability to work within an inter-professional framework and collegiate manner.
- Demonstrated leadership skills.
- Excellent at developing, establishing, and sustaining interpersonal relationships.
- Genuine desire to participate in the provision of care to frail and aged people.
- Demonstrated Knowledge and understanding of the Aged Care Quality Standards.
- Knowledge and understanding of AN-ACC assessments will be highly regarded.
- Current or willing to obtain a NDIS Clearance Certificate
Responsibilities:
- As the Clinical Nurse you will be responsible for reviewing Support plans and coordinating with the Admissions teams and Clinical Governance Manager for new admissions.
- You will also be responsible for coordinating with the Clinical Team with respect to clinical assessments to ensure timely completion of resident care plans and ongoing evaluation.
- Supports and promotes the Facility’s philosophy and objectives, Policies and Procedures.
- Works in accordance with the Code of Ethics for Nurses and regulations as required by the Australian Health Practitioners Registration Authority.
- Demonstrates knowledge and application of the Aged Care Quality Standards.
In collaboration with the Executive Team and the Clinical Governance Manager onsite you will:
- Review Support plans for applications for resident admissions.
- Coordinate and liaise with Clinical Governance Manager.
- Support Workers and Admissions Teams to ensure seamless clinical administration at admissions of new residents.
- Review care evaluations for content to ensure residents care issues are identified and clinical care requirements are addressed.
- Responsible for coordinating with Registered Nurses for timely completion of clinical assessments.
- Responsible for monitoring Care plans reflect ongoing changes in acuity and care needs of residents.
- Clinical incident management and reporting
- Responsible for finalizing Clinical incidents.
- Responsible for Weekly S8 Management check, clinical indicator performance, evaluation and reporting
- Infection prevention and control within clinical services
- Provide support and direction to Clinical Team (Registered Nurses and Enrolled Nurses) with addressing gaps in clinical risks identified.
- Lead the Clinical agenda at RN Meetings.
- Responsible for following up on Residents returning from hospital with recommendations follow up, medical diagnosis updated and discharge summary and uploaded on CMS.
- Contribute to the Clinical Care Committee.
- Assists the Clinical Governance Manager with collating Government NQIP (National Quality Indicator Program) data are analysed, improvements documented and action and submitted in line with Residency by Dillons’ timeframes.
- Responsible for ensuring Allied Health referrals have been made as recommended and Allied Health recommendations followed up (e.g. Physio to review resident post fall).
- Support the Clinical Governance Manager and Executive Team for external regulator reviews/ audits relevant to Aged Care and communicate any improvement activities that may be required.
- Support with meeting accreditation with Aged Care Quality Standards.
- Training and Supervision of Staff In conjunction with the Clinician Governance Manager, assists in the provision of education and training for staff commensurate with experience.
- Responsible for orientating new Clinical staff.
- Provided ongoing support to staff.
- Identifies performance gaps and learning needs of support staff in conjunction with the Clinical Governance Manager and Executive Team in order to maintain a high level of staff competency.
- Act on any safety risks such as staff incidents, accidents and hazard reports to maintain a safe working environment.
- Act in accordance with the facilities current Infection Control Policies and Procedures including the identification and management of infectious outbreaks.
- Participates in the formal continuous improvement program that aims to improve the quality of all services delivered.
- Attend required meetings to actively contribute to the planning and development of the service.
- In collaboration with the Executive Management Team and Clinical Governance Manager participates in own areas of professional & personal development, including performance appraisal process.
- Attend all mandatory education sessions as determined by the Executive Director of Quality and Marketing. Maintains a professional level of personal hygiene, appearance and behaviour.
