Responsibilities:
- Provide high-quality care to elderly individuals in the comfort of their homes
- Conduct comprehensive assessments and develop individualised care plans
- Administer medications and perform necessary medical procedures
- Collaborate with other healthcare professionals and family members to ensure holistic care
- Maintain accurate and up-to-date documentation of patient records
Requirements:
- Registered Nurse qualification with current AHPRA registration
- Previous experience in Aged Care or community nursing preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a collaborative team
- Compassionate and patient-centred approach to care
- Must have own transport
