About the role
The short stay Adult ‘step up, step down’ Prevention and Recovery Care (APARC) program, delivered in partnership Victorian hospital networks and Health Service, supports clients aged 25-65 years with psychosocial disability who require individualised treatment and services when entering from the community or transitioning from acute inpatient care. PARC provides recovery-focused, clinical treatment frameworks to support clients achieve their recovery goals, enhance connections with families and carers, build confidence and social skills, and increase capacity to live in the community.
We are seeking an empathetic, self-motivated Community Mental Health Practitioner for a casual opportunity to work alongside a caring team on a 24/7 rotating roster in the Warrnambool area. You will provide clients empathetic supports and short-term intensive treatment to achieve their recovery goals and live independently.
Key responsibilities
- Support clients to return to their community and enjoy a meaningful life through an individualised approach to recovery.
- Deliver high quality services and facilitate positive educational and social experiences.
- Work as part of a multidisciplinary team offering specialised psychosocial support.
- Plan and deliver group programs that aid clients to build their skills and focus on recovery.
- Collaborate with a range of services and the community to ensure a co-ordinated, integrated response to the client’s recovery goals.
What you’ll bring
- Tertiary qualifications (minimum Certificate IV) in Mental Health, Psychology, Social Work, Occupational Therapy or other health-related field.
- Expertise in working with people with mental health issues, complex needs and interacting with families and carers.
- Relevant experience in facilitating group activities for clients experiencing mental ill health.
- Demonstrated understanding of the Community Services sector and ability to build strong relationships with service providers, clinical teams and key stakeholders.
