Complex Health TeamCareer Opportunity Expression of interest  

Fina are launching the first Complex Health Team in Woombye to deliver support to a client with a disability and complex medical health conditions.   

We will be hosting a meet and greet to discuss this role on Wednesday 20th March at 05:30pm at Woombye for you to come along and ask questions.  

Our clients have individual complexities that include Diabetes Type 1 and 2, Dysphagia and complex mealtime supports, Epilepsy, and chronic health related illnesses.  Each client has an individually designed high intensity support plan developed by our subject matter experts in consultation with medical and allied health professionals.   

This role offers: 

  • Training and competency assessments of the High intensity supports individually designed by Fina and delivered by a Registered Nurse.  
  • Regular support from Fina’s Registered Nurse 
  • Team collaboration and analysis health condition trends through methodical review processes. 

Pay, conditions and work/life balance 

  • As a Complex Health Team member, you will be on a salary of $72,800.00 
  • As a Permanent Part time Salaried worker, you will receive 7 rostered shifts (56 hours) with an ability and expectation to pick up 2 shifts when required per fortnight.  
  • Equally share all shift types with your team working a 6-week roster rotating each fortnight across a set of Mornings, afternoons, sleepover/awake shifts as per the service requirements.  
  • Line partner arrangement to provide self-managed flexibility for approved shift swaps.

Qualifications and/or experience in Individual Support, Disability Support , Complex Care medical support and Mental Health are desirable - but not necessary.

  • Exhibit composure and demonstrate empathy towards the client and other members of our support team. 
  • Exercise active listening while interacting with the client and other members of our support team in all scenarios. 
  • Ability to navigate Complex Health supports in a professional manner 
  • Use effective communication in all situations using clear, concise, and appropriate / assertive language when needed to convey messages and manage emotions. 
  • Collaborate with client and team members to coordinate the complex care requirements of the client and team is upheld. 
  • Provide guidance and support to colleagues, clients, and the public to manage complex care supports.  
  • Ability to construct concise reports and document incidents as they appear, including recommendations and follow-up measures. 
  • Adhere to policies, procedures, rules, and regulations. 
  • Actively contribute to key elements of excellence in supports. 
  • Participate in ongoing training to maximise efficiency in the role. 
  • Maintain physical and emotional fitness  
  • In a multi-disciplinary complex care environment.  

Mandatory Requirements   

  • QLD Driver License  
  • 2 x COVID vaccinations   
  • A current Blue Card   
  • A current Yellow Card   
  • Current First Aid & CPR
