- Inpatient care across nine wards at Kingston Centre and Casey Hospital, with a total of 253 beds.
- Inpatient care in patients’ homes (Monash at Home) with 25 beds to expedite ward discharges from our hospital campuses and a phased expansion to 40 beds by November 2023, to cover the entire Monash Health catchment.
- Community specialist clinics that care for priority populations (Movement Disorders, Young Adult Transition Service, Cognitive and Dementia Memory Service, Clinical Gait Analysis Service, Falls and Balance Clinic, Pain Clinic, Continence Clinic, National Disability Insurance Scheme)
- Community Rehabilitation services across the full catchment (five centres) and offer time-limited interdisciplinary care to promote independence within our communities. Services are offered at home, at one of our centres and/or by tele-rehab consultations, and can be delivered in a group or individual setting.
- Commonwealth-funded aged care programs in the community, including the Aged Care Assessment Service (ACAS), Regional Assessment Service (RAS), Home Care Packages (HCP) and Commonwealth Home Support Program (CHSP) and Transition Care Program (TCP). TCP provides short-term care after a hospital stay with 48 contracted care beds through a private residential aged care facility, and an additional 35 places to support clients in their own homes.
- Support and assist the Program Medical Director of Aged and Rehabilitation Division and Head of Neurology, Monash Health in the provision of high-quality Movement Disorders Services at both Monash Medical Centre and Kingston Centre, and other sites within Monash Health, consistent with the Monash Health values.
- Provide clinical leadership through active involvement in patient care.
- Be available to medical colleagues for consultation and liaison as appropriate regarding patient care.
- Support academic, research, education, training & supervision (both undergraduate and postgraduate) roles at Monash Health.
- Contribute to the professional and corporate life of Monash Health by participation in appropriate committees and fora.
- Contribute to ongoing service improvement and clinical and workforce innovation to ensure Monash Health provides high quality accessible Geriatric Medicine and Neurology Services within allocated resources and meets the needs of Monash Health patients.
