About The Role:

The Continence Coordinator is responsible for the planning and coordination of resident continence care and ensuring focused delivery of their specialised understanding of all aspects of continence with our other clinical team members.
In addition, it is expected that the Continence Coordinator will actively participate in the training and education of all staff employed by The Shoreline relating to the best practice of all aspects of continence management including but not limited to urine charts, skin integrity assessments, colostomy assessments and use of Clinical Manager.

In this role, you will be responsible for:

1. Provide and/or coordinate the delivery of accountable and responsible person centred, outcome-based nursing care that reflects current standards of nursing practices and consistent with The Shoreline’s policies and procedures.
2. Maintain current knowledge and clinical proficiency in nursing care, in accordance with best practice, legal and professional standards
3. Assist care and clinical staff with the coordination and delivery of continence services, together with the management of resident supplies
4. Ensure formal lines of communication are followed and maintained, including reporting changes in continence needs to the Registered Nurse team.
5. Report to and collaborate with facility-based management (Director of Care & Deputy Directors of Care) to ensure the continence needs of residents are met.
6. Meeting and liaising with suppliers to ensure appropriate products are utilised in order to maximise resident outcomes in the most cost-effective way.
7. Utilise a problem-solving approach in order to assist with the provision of effective nursing care.
8. Act as resident advocate by ensuring the rights, safety, privacy and confidentiality of residents is maintained.
9. Foster harmonious and effective communication and relationships with residents, their families and other persons, as appropriate.
10. Actively encourage and support each resident’s sense of independence and the exercise of choice, particularly with respect to continence needs.
11. Ensure all continence assessments and related documentation, including charts and care plans are completed in a professional and timely manner, so as to meet the objectives of good clinical care, accreditation and funding requirements.
12. Be familiar with emergency procedures and equipment and use appropriately
13. Provide appropriate education and support to staff, residents and families in relation to continence

PURCHASING
14. Source and purchase materials, goods, and services for the clinical space
15. Responsible for creating and maintaining relationship with new and existing suppliers
16. Monitoring and maintaining clinical stocks
17. In collaboration with management, evaluating suppliers before making purchasing decisions, negotiating terms and pricing
18. Tracking orders

About You:

As someone with a proven track record of compassion, you will demonstrate kindness and empathy with the elderly and other diverse individuals. You will possess an intuitive responsiveness to aged care residents' individual wants and needs, whilst maintaining calm, composed and optimistic when faced with stressful or high pressure situations. You are motivated by the opportunity to contribute to bettering the life of our aged care residents through assessment and management of continence and you are driven to be a part of a facility that truly aims to set a new standard.

To succeed in this role you will need the following skills and attributes:

• Minimum of a Diploma of Nursing qualification and current registration with AHPRA as an Enrolled or Registered Nurse.
• High level knowledge of Continence Management, supported by relevant training and/or study
• High level communication skills including report writing and experience in writing procedures
• Advanced knowledge of aged care legislation, regulations, and accreditation
• High level of analytical skills of quality indicator data
• Ability to work in a multi-skilled team environment and to communicate effectively with management, staff, and stakeholders, including medical and allied health professionals
• The ability to educate/mentor
• Highly developed organisational skills and ability to determine work priorities
• Understanding of the ageing process and a commitment to advocate for older people in residential care
• Ability to use Microsoft Office software and other desktop applications to an intermediate level
• Police check clearance
