Coordinator of Supports – Disability Services
Based inMelbourne, VIC
Salary Range: 82K to 90K + Super (Based on merit)
We have an exciting opportunity for a Coordinator of Supports (COS)to join our vibrant and transdisciplinary team on a permanent role based inMelbourne.
About the Role
The Coordinator of Supports (COS)is responsible for ensuring capacity building support for the participants ofas per their individual plan including informal, mainstream, community andfunded supports.
This role is responsible forproviding support within a specialist disability framework necessitated byspecific high-level risks in participant’s situation. The support to beprovided is time limited and focuses on addressing barriers and reducing complexityin the support environment, while assisting the participant to connect withappropriate supports, building capacity and resilience. It may also involvedevelopment of an intervention plan which is to be put in place by disabilitysupport workers.
On a daily basis, this position will be accountable formaintaining stakeholder relationship, implementing participant’s plan, ensuringparticipants have greater opportunities to explore and connect with thecommunity and alternative support options. This position will promote Communityinclusion, independence, decision making and personal choice by ensuringsupport, with a focus on individual goals.
In this role, you will be working within a transdisciplinary teamand will be responsible for:
· Assess a number of mainstreams,community, informal and provider options
· Choose preferred options orproviders
· Negotiate services to be providedand their prices, develop service agreements and create service bookings withpreferred providers
· Arrange any assessments requiredto determine the nature and type of funding required (eg. assessment todetermine the type of complex home modifications required)
· Decide the budget for each supporttype and advise any relevant plan manager of the breakdown of funds
· Liaise with any plan manager toestablish the appropriate claim categories and attribute the correct amount offunds
· Link to mainstream or communityservices (i.e. housing, education, transport, health)
· Strengthen and enhance theircapacity to coordinate supports, self-direct and manage supports andparticipate in the community, including providing participants with assistanceto:
o Resolve problems or issues thatarise
o Understand their responsibilitiesunder service agreements
o Change or end a service agreement
· Assisting the participant to getready for their plan review by helping them:
o Assess whether they achieved theirgoals and got value for money for their plan
o Identify solutions to problemsexperienced in implementing the plan
o Consider new goals
· Helping participants decide onwhat actions to take to achieve goals in relation to exploring housing optionsand life transition planning.
· Rostering appropriate staff tosupport participants where applicable.
· Develop programs for participantsto access activities and experiences based on their needs and interests.
· Ensuring compliance with staffing,funding and reporting requirements.

Aboutyou
Tobe successful in this role, you will need:
· ATertiary qualification in Social work, Community Services or Counselling.
· Atleast 4 years’ experience ins servicing NDIS clients
· Asound knowledge of NDIS Framework and provision of Disability supports services
· Experienceworking within a team environment implementing transdisciplinary practice
· Abilityto interpret professional reports
· Fluencyin Spanish will be an added advantage
