Coordinator Transitional Care

This is a Fixed Term Full Time appointment until 30/06/2026, with the possibility of extension(s) and/or permanency.

Whilst this selection process will initially be used to fill the above vacancy, it may also be used to fill other "similar" vacancies throughout the health service. In addition to this, should the successful applicant decline or vacate the advertised vacancy, then the next most suitable applicant may also be selected from this process. Both of these options remain valid for a period of twelve (12) months from when the authorised delegate endorses the recruitment decision.

Position Profile: Coordinates the operations of the Transition Care Program and Short Term Restorative Care to ensure appropriate planning, management, accountability and delivery of high quality services to the allocated region. Provide advanced clinical skills to direct assessment and care of the clients within the multi-disciplinary team, in collaboration with senior practitioners. Support theimplementation of the strategies, principles and practices of the Sub-acute care Plan and National Action Plan - Older Patient Initiative.
