About the role: Disability Support Worker
The Disability Support Worker is responsible for the provision of person-centered care and support and may work independently with a client or alongside our team of Support Workers at a central location. In partnership with clients and other health professionals, Disability Support Workers develop and implement programs and services aimed at achieving an individual’s goals and aspirations.
Support Workers mostly assist clients with the following tasks: domestic assistance, meal preparation, medication assistance (where qualified), personal care, shopping and social support, transport, and possibly sleep-over support. Tasks need to be carried out by the Support Worker with client choice and client centered service at the heart of what we do. You will be required to complete logging of time appropriate to client's care plans to enable us to deliver transparent and appropriate management of our client's care, administration of plans and billing of services
KEY SELECTION CRITERIA

- Satisfactory completion of the relevant Disability Support Certification/Qualification, or the ability to demonstrate enrolment, commitment to undertake and satisfactory progress toward completion.
- Previous experience in relevant industry, health or personal care related services highly desirable
- Excellent customer relations skills, with the ability to preserve the autonomy, rights and dignity of our clients in a timely, safe and professional manner
- High level verbal and written communication skills, with the proven ability to complete administration associated with NDIS care provision to our clients and the transparent keeping of records
- Demonstrate a high level of skill in dealing with clients with complex needs in a supportive, empathetic and effective manner
Other Relevant Information Employment Eligibility
- Applicants must be an Australian Citizen, Permanent Resident or hold a valid work permit or visa
- Applicants will be subject to a probation period of six months
All new appointments will be subject to a NDIS Safety Screening activity, including (but not limited to) any provisions required under the Disability Worker Exclusion Scheme;
- National Police Records Check which will comply with the 100-point Proof of Identity Check
- Current and valid Drivers License Check
- Two professional reference checks,
- International Police Check (if applicable) and
- Working with Children check (if required)
