Form of workAn exciting opportunity exists for a Discharge Planner to join our dynamic team at Hobart Private Hospital.
As Discharge Planner your responsibilities will include:
- Act as a resource, advising on relevant matters relating to the discharge planning process and episode management
- Liaises with Unit Managers, nursing staff, allied health Professionals and Medical Practitioners to coordinate patient discharge plans
- Ensures information relating to patient discharge plans is documented and accessible by all clinical staff.
Selection Criteria:
- Demonstrated effective communication, interpersonal and team building skills
- Must have knowledge of local services both publicly and privately to ensure safe and effective discharge of patients
- Relevant experience in this area of nursing essential
- Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
- Commitment to personal and professional development
- Good Interpersonal and time management skills with an ability to work within a multidisciplinary team
- Demonstrated ability to deliver high quality patient care
- Supports and facilitates the patient and family to make informed decisions by providing appropriate information and options regarding health and nursing care
- Social Worker (desirable)
