Address An exciting opportunity exists for a Discharge Planner to join our dynamic team at Hobart Private Hospital.
As Discharge Planner your responsibilities will include:
Act as a resource, advising on relevant matters relating to the discharge planning process and episode management
Liaises with Unit Managers, nursing staff, allied health Professionals and Medical Practitioners to coordinate patient discharge plans
Ensures information relating to patient discharge plans is documented and accessible by all clinical staff.
Selection Criteria:
Demonstrated effective communication, interpersonal and team building skills
Must have knowledge of local services both publicly and privately to ensure safe and effective discharge of patients
Relevant experience in this area of nursing essential
Current Registration with the Australian Health Practitioner Regulation Agency (AHPRA)
Commitment to personal and professional development
Good Interpersonal and time management skills with an ability to work within a multidisciplinary team
Demonstrated ability to deliver high quality patient care
Supports and facilitates the patient and family to make informed decisions by providing appropriate information and options regarding health and nursing care
