Duties
- Provide a high quality, comprehensive service to inpatients of the hospital to facilitate timely and effective discharge planning.
- Conduct thorough initial assessment - establish previous and current level of function, supports and services.
- Work collaboratively in a multidisciplinary team to deliver quality patient outcomes.
- Implement and coordinate a timely plan for discharge, liaise with community service providers and organise home support where relevant and required.
- Provide information, education and support to patients and families.
- Current registration with AHPRA as registered nurse.
- Minimum of 3 years' experience in an acute health care setting.
- Excellent interpersonal and communication skills.
- Ability to work autonomously and also as a team player.
- Ability to problem solve and negotiate with patients and carers to achieve optimal outcomes.
- Adequate clinical knowledge to understand complex patient cases.
- Excellent time management and multi-tasking skills.
- Sound knowledge of My Aged Care system and ACAT referral criteria.
- Commitment to personal development and maintenance of current evidence based practice.

About Us
John Flynn Private Hospital is a 354 bed, acute care hospital located at Tugun, which is on the southern end of the Gold Coast, within 5 minutes from the Gold Coast Airport.
