Your part in Buderim Private Hospital

The Clinical Care Coordinator / Discharge Planner is responsible for providing assistance to the departments/wards by facilitating a seamless end to end experience for our patients and their care. The role includes: 

• Ensuring discharge planning is undertaken in a timely and coordinated manner to optimise patient outcomes and length of stay.
• Develop and maintain open communication with key stakeholders to coordinate patient discharge plans
• Assist in the development of documented resources in order to assist clinicians in the management of discharge planning (including; referral process, policies/procedures and clinical pathways).

What makes a difference for us?

• Bachelor of Nursing Degree or equivalent
• Current AHPRA registration 
• Post-graduate qualifications relevant to the area of practice (desirable only)
• Highly developed communication and interpersonal skills
