- Participate in care planning and assessments as directed the Clinical Care Coordinator/Care Coordinator or Registered Nurse
- Engage in collaborative practice to achieve positive outcomes for the resident
- Coordinate the delivery of quality clinical services as directed by the Registered Nurse to meet individual resident needs
- Coordinate communication between the care staff, Registered Nurses and relevant health professionals to ensure continuity of care to the residents. This is achieved by ensuring discharge summary admission notes and other correspondence are included in handover and communicated to residents, families and relevant staff as required.
- Diploma of Enrolled Nursing/ Current AHPRA Registration
- Medication Endorsement
- Knowledge and understanding of the Aged Care Funding Tool (ACFI)
- Commitment to continuous quality improvement
- Demonstrated empathy and patience with the older person
- Demonstrated high standard of verbal and written communication skills
