About the Role:Provide leadership and supervision to Personal Care Workers and support Registered Nurses in their role to promote a harmonious team approach to the delivery of resident care at Box Hill facility.
- Deliver high quality, person centered care to residents and families, including consideration for cultural, spiritual and individual needs
- Provide nursing care to residents with complex needs such palliative care, dementia support, restorative care and pain management to enhance their wellbeing
- To support nursing staff regarding assessment and care planning processes, documentation and evaluation of care outcomes
- Analyse risk, implement strategies and participate in clinical handover with Registered Nurses
- Promote a harmonious team approach by acting as a role model in coordinating daily routines and adhering to safe practice measures in medication management in consultation with the Registered Nurses
We currently have two Permanent Part-Time shift Afternoon shift lines:14:45pm to 22:00pm:
- Wednesday, Thursday, Friday and Saturday (Week 1 & 2)
- Sunday, Monday, Tuesday and Wednesday (Week 1 & 2)
About you:You are passionate about what you do and want to make a difference in the lives of our residents to support them to live life to their potential!
- Current registration with AHPRA as a Medication Endorsed Enrolled Nurse (Div 2)
- Experienced as an EEN in residential aged care/ home care (preferred)
- Experience with dementia care and palliative care support (desired)
- Proficiency in decision making, critical analysis and on the spot problem solving
- Demonstrated abilities to delegate and prioritise in a fast paced environment with exemplary communication skills in a team and one on one interactions with all levels of care partners
- Available and committed to work in the above mentioned roster vacancies
- Thrive in a team and also appreciate the one to one interactions
