Job description

This position is identified for an Aboriginal and/or Torres Strait Islander person and is intended to constitute a special measure under section 8 (1) of the Racial Discrimination Act 1975 (Cth), and section 57 of the Anti-Discrimination Act 1996 (NT).

Who are we?

Established in 1991, Danila Dilba Health Service (DDHS) is an Aboriginal community-controlled organisation committed to providing culturally appropriate, comprehensive primary health care services and community development within the Biluru (Aboriginal) community of the Yilli Rreung (Greater Darwin) Region in the Northern Territory.

We are focused on acute care, immunisation, chronic disease management, women's health, men's health, child and maternal health, sexual health, and social emotional wellbeing.

We also provide a range of community programs in areas including health promotion, mental health/social and emotional wellbeing, and alcohol, tobacco, and other drugs. 

About the role

The Healing and Recovery Service is a proactive Aftercare service that is designed and delivered to support Aboriginal and Torres Strait Islander People following a suicide attempt or suicidal crisis.  The Healing and Recovery Service provides a safe environment, culturally appropriate care co-ordination, support and linkages with different aspects of treatment or response.

Responsibilities:

  • Provide high quality aftercare services to Aboriginal and Torres Strait Islander individuals who have been referred to DDHS Healing and Recovery following a suicide attempt or suicidal crisis
  • Initiate, coordinate and support the planning, development and implementation of culturally appropriate suicide prevention, intervention and postvention activities with clients develop individually tailored care and safety plans with clients
  • Provide 3-months follow-up check-ins as required to support people to remain linked with their appropriate support networks to facilitate ongoing care
  • Monitor and assess clients status to implement changes to care plans to ensure appropriate supports are in place
  • Establish a strong therapeutic alliance with clients ensuring continuity of care, including engagement with the individuals GP, or in the case where there is not a regular GP, linking them with one
  • Work in partnership with relevant government and non-government service providers and community groups to support clients in creating a support network, reduce distress and aiming to prevent re- presentation to Emergency departments
  • Maintain strong referral pathways, including but not limited to the following services: acute care services, primary care services, schools, police, Alcohol and Other Drugs Services mainstream and Aboriginal and Torres Strait Islander-led mental health services.

To be successful you will need:

Essential

  • Cert IV Mental Health/Cert IV in Aboriginal and/or Torres Strait Islander Primary Health Care or equivalent training/ work experience or lived experience.
  • Excellent written and oral communication skills.
  • Demonstrated understanding of relevant legislation and policy frameworks.
  • Experience in the delivery of culturally safe care
  • Client focused with an ability to advocate on client's behalf.
  • Ability to multi-task, prioritise workloads and meet deadlines.
  • Proficiency in computer software including electronic medical software and the Microsoft Office Suite.
  • Ability to work independently and in a team environment.
  • 2 or more years of relevant work experience

Desirable

  • Experience working in suicide prevention.
  • Experience working in aboriginal community-controlled sector.

What we offer:

  • 6 weeks annual leave per annum.
  • 5 days paid leave during our end of year shut down period (in addition to annual leave).
  • Salary packaging of up to $15,990 per annum.
