Job description

This position is required to support CRCS’s Program in planning and delivering a range of services to ageing participants, to support them to maintain capacity for independent living, and to support their carers. This position will also facilitate services for people who require in home support, individual social support, transport, group social support, allied health supports and other supports as required, including brokered services.

Reporting to the Program Manager Aged Care, the Case Worker is a skilled communicator, well organised and has acute attention to detail with excellent timekeeping and case note accuracy. Case Workers are passionate about working with older people to achieve their goals and live their best life. 


Key responsibilities include: 

  • Assists eligible older people to identify, plan and implement strategies to achieve their personal goals within a person-centred framework.
  • Develop trusting, positive and professional relationships with Participants and service providers.
  • Provides information to Participants and advocates when necessary to facilitate access to community services and facilities
  • Liaises with carers, families and significant others involved in providing care, including specialist generic agencies and other mainstream service providers.
  • Works collaboratively with other Case Workers within the team.
  • Assists with problem solving and addressing any emergent issues
  • Identifies any communication or workflow issues and makes suggestions for improvement.
  • Carries out administrative tasks, including entering accurate case notes in a timely manner and maintaining files and statistics as directed
  • Completes regular progress reports within required timeframes.

CRCS also requires all employees to:

  • Hold or be willing to obtain a satisfactory Working with Vulnerable People registration (ACT) and/or a Working with Children Check (NSW), and have these with them at all times whilst employed by CRCS.
  • Undertake a National Criminal History Clearance by CRCS prior to commencing and every three years thereafter as a condition of employment. Employees are required to advise CRCS of any convictions that may effect their eligibility to work or perform the inherent requirements of their position 
  • Maintain full COVID-19 vaccination currency and provide evidence of their vaccinations to CRCS (sensitive information is kept private and in accordance with the Australian Privacy Principles) unless they are able to provide suitable evidence of a medical contraindication that prevents them from receiving this vaccine.
