We are currently looking for a passionate, nurturing community service professional to join our Regional Assessment Service as a Home Support Assessor based in Cairns.
Your role:
As a Home Support Assessor, you will be mobile, flexible and skilled in assessment to ensure client choice and control in the process of assessment, goal setting, service selection and review.
Using your skilled assessment techniques, you will support each client individually, as the level of support may vary from client to client with different physical, social, emotional and clinical needs. You will also focus on client and consumer direction for appropriate care.
You’ll have or demonstrate:
- A Certificate III level qualification in a Community services Field (Such as Allied Health Assistance, Disability, Aged Care, Community Services and/or other related discipline)
- Completion of the My Aged Care & NSAF Systems training or willingness to obtain
- Previous experience working within aged care and/or community services
- QLD Driver’s License and roadworthy registered vehicle (Substantial travel will be required from time to time)
- Experience with technology incl. smartphones, tablets, laptops etc.
- A high level of verbal and written communication skills
