Home Support Association Inc (HSA) originated as a local, grass-roots service through the work of people living in this community and driven by the needs of people living in this community and continues to operate under these principles today after more than 30 years of change and growth.
At HSA, as a truly local, NDIS provider, we work alongside people to identify their strengths, interests, and goals, providing just the right amount of support they need to be able to live the lifestyle they choose.
The role:
As a Supports Coordinator, you will work directly with people living with disability to implement their NDIS plan and link them with service providers to ensure their support needs are being met. You must have an excellent work ethic, strong time management skills, and attention to detail. You will manage your own caseload of NDIS participants, while using your strength-based, person-centred approach to establish rapport and promote professional relationships.
We are looking for someone who is passionate about supporting the rights of people with a disability and working collaboratively with mutual trust and respect. You will need to have effective problem solving and decision-making skills and the ability to schedule your work to meet deadlines in a busy environment. Our ideal candidate will have excellent communication skills and be able to effectively build and maintain positive relationships with a broad range of stakeholders.
Our ideal candidate will have:
- Proven ability to work effectively within a team, establish work priorities and meet deadlines in a fast-paced environment
- High level of computer skills including sound knowledge of the MS Officer Suite and corporate software systems
- Well-developed oral and written communication skills with high level of accuracy and attention to detail as well as tact, diplomacy, and confidentiality in dealing with a variety of external agencies and service users and their families and/or guardians
- Experience in the provision of a client directed strength-based approach with person-centred services
- Knowledge of, and connections with disability, and community services within the Capricorn region
- Current ‘C’ class driver’s licence
- Current Working with Children Check (blue card) and an NDIS Worker Screening Check (yellow card) or ability to acquire.
- Minimum 2-3 years’ experience working within a support coordination capacity within the NDIS framework
