Local Government position- Northern Suburbs
3 months assignment with an award winning local government organisation that prides itself on a customer focused culture of innovation, best practice and continuous improvement.
As part of the Health, Aged and Community Planning Division your responsibilities will include :
- Intake,Assessment and review of clients under the HACC PYP Program
- Undertake broad holisitic assessments in the clients home
- Identify clients needs and set realistic goals, in keeping with individual cultural and diversity requirements
- Accurate recording of client information using Xpedite system
To be successful in your application you will need:
- Tertiary qualifications in Nursing, Social Work, Allied Health or a related field
- Demonstrated previous experience in assessments, particularly within HACC PYP
- Ability to collect information in a sensitive manner, with particular regards to cultural requirements, language issues, special communication needs
- Ability to manage and prioritise a variety of complex tasks concurrently with minimal supervision
- A current National Police check
- A current Visctorian Working with Childrens Check
