About Kingston Aged Care Assessment Service & Regional Assessment Service
ACAS and RAS Programs provides comprehensive, holistic assessments as part of My Aged Care for frail older persons. The assessment considers the physical, medical, psychological and social needs of the client, an evaluation of their functional potential and develops care plans that promotes restoration of function and maximises independence. The ACAS and RAS programs links people in with Commonwealth Home Support Program, Private services and delegable Commonwealth funded services such as residential respite, permanent care, Home care packages and Flexible care options.
The Regional Assessment Service (RAS) is responsible for providing Home Support Assessments to assess people for eligibility for entry level supports that will enable older people to continue living independently in their home. RAS is part of the broader department with ACAS & TCP at Monash Health.
Monash Health is seeking an experienced Home Support Assessor to join our RAS team.
We provide ongoing training, professional development, and a supportive team environment to ensure our staff can achieve their best.
