Practice Lead

Full Time

Who are we?

Wintringham is a not-for-profit organisation providing an extensive range of aged care, housing, and related support services to people aged over 50 years old, who have been homeless or at risk of homelessness.  We employ over 1000 staff and operate from multiple offices across Melbourne and regional Victoria and Tasmania.

Your new role

This is an exciting opportunity for a professional experienced with Intensive Case management eager to demonstrate their exceptional leadership skills within our HHS Team. In addition to managing a small caseload of HHS clients with complex needs, this role will provide leadership, practice guidance, and support to a team of HHS Case Workers.  As a point of escalation, you will manage issues and incidents on-site and ensure effective communication with key stakeholders and the Site Coordinator. This role will work collaboratively with clients to develop individualized case plans through identifying their strengths, as well as their support needs.

Your responsibilities and duties will also include:

  • Work closely with the HHS Co-Ordinator to provide staff supervision to staff managing client a caseload requiring low to medium intensity outreach-based support.
  • Actively participate in team meetings, supervision, reflective practice, annual performances appraisals and practice development across the portfolio.
  • Work closely with the Senior Tenancy Worker
  • Work collaboratively with residents to create a meaningful life, assisting them to develop self-management skills across mental and physical wellbeing, personal safety, and crisis management and harm minimisation.
  • Work with residents by linking and referring them into clinical services as well as social and recreational activities.
  • Utilise your knowledge of harm reduction and brief intervention, trauma informed practice, conflict resolution, MARAM and Housing First principals to manage a case load of clients.
  • Ensure residents obtain access to Commonwealth Home Support Program (CHSP), Home Care Packages and NDIS services by providing referrals and case management services.
  • Responding to resident feedback and complaints in line with Wintringham policy and procedures.
  • Utilising relationships within sectors to assist access into other services and systems.

We are looking for candidates with the following:

  • MUST have a tertiary qualification in Social Work, Welfare, Community Services, or equivalent
  • Experience in a senior practitioner role with specific training and/or experience in: Housing, Mental Health and Family Violence or Homelessness services.
  • An understanding of the Housing and Homelessness Service sector My Aged Care, NDIS and other relevant service systems.
  • Working knowledge of social housing and tenancy related matters.
  • Knowledge of, or the ability to acquire regarding the Residential Tenancy Act, 1997 TAS
  • Proven commitment to excellence, accountability, and innovation in best industry practice in service delivery in the fields of homelessness and social housing support.
  • Well-developed communications skills and the ability to with engage people from diverse backgrounds.
  • CPR and basic First Aid Certificate.
