Housing Support Worker (HSAP program)

Part Time

Who are we?

Wintringham provides aged care services to those who are financially disadvantaged and have previously been homeless or are at risk of becoming homeless. We employ over 950 staff and operate from multiple offices in Melbourne, Hobart and regional Victoria. Wintringham is a major provider of home care. Our team of Carer’s, Case Managers, Outreach Workers, Care & Support Workers and Recreation staff provide holistic care and support to clients who live independently in their own homes. The care provided to Wintringham clients is carried out with the underlying principles of social justice

Your new role

We are currently seeking a motivated and enthusiastic person, with a commitment to social justice, to join our multi-disciplinary community team at Northern Community Housing and Support Office as a Part Time Housing support Worker on a part time ongoing basis for 30 hours per week, working out of the Northern Office. This program has been developed to support these clients to maintain stable long term housing and to improve their health and wellbeing.

Your roster will be as follows:
Week 1 & 2: Monday, Tuesday, Thursday & Friday (0900-1700)

Your duties and responsibilities will include:
  • Engage and establish a relationship of trust with clients.
  • Assist the Manager and Housing Staff prioritise and decide who is nominated into any vacant properties.
  • Identify and conduct ongoing client needs assessments on the unmet care and support needs of clients, taking into account their circumstances and preferences.
  • Assist Clients where necessary, to move into accommodation with Wintringham or Housing Choices Australia, including co-ordinating relocation tasks as required.
  • Regularly visit and monitor the health and welfare of clients.
  • Provide practical and immediate assistance to clients to overcome health or other crises.
  • Develop, implement, and review Case Files and assessments for each client.
  • Co-ordinate the delivery of service to clients and advocate on their behalf if required.
  • Monitor the degree to which client needs are being met by the program and other service providers.
  • Develop and implement range of social support strategies and provide practical assistance to link clients into services that meet their social support needs.
  • Provide case management to co-ordinate client access to services.
  • Provide practical and flexible assistance such as making and helping clients to get to appointments and social support.
  • Assist in enhancing responsiveness of local service providers to the needs of the target group and the achievement of sustainable improvements in the quality of service delivery.

We are looking for candidates with the following:
  • A relevant post-secondary qualification and experience relating to an area of health and/or community services practice – for example, Certificate IV in HACC/Aged Care, Welfare Studies, Case Management or Social Work.
  • Experience in Aged Care
  • Knowledge of housing and support issues facing older homeless people who may have a physical and/or mental disability.
  • Skills in assessing clients care, support, health and housing needs.
