CTST is a vibrant and growing not-for-profit charitable community organisation whose vision is that everyone has access to community transport. We specialise in providing reliable, high quality and safe transport to older people and people with a condition preventing them from undertaking daily activities.
We're seeking someone with a strong knowledge of aged care governance, NDIS, or someone with a background in occupational therapy or nursing who can work collaboratively to review and enhance existing planning, intake and assessment processes.
The successful candidate will have the opportunity to positively influence the aged care sector to re-design and operationalise a future-focused intake and assessment framework (CHSP, HACC and NDIS) to deliver high quality consumer directed services in line with ongoing aged care reforms.
The position is a full-time position and is based at our office in Glenorchy office. The role is classified at Level 6 of the Social Community Home Care and Disability Services Award 2010 (SCHADS) with the benefit of salary sacrifice options.
About the Position
The successful candidate will be required to complete the following duties:
- Collaborate with key internal stakeholders to review existing intake, assessment, and planning processes, and
- Conduct research and make recommendations in relation to best practice intake and assessment standards
- Design and implement a comprehensive intake, assessment, and planning framework
- Utilise project management skills to develop a clear project plan, key milestones and change and communications plans
- Provide end to end development and implementation of service and process improvement
- Develop ongoing intake, assessment, and planning strategies to proactively support CTST’s response to future changes in aged care reform
- Drive a connected and enhanced consumer experience across all touchpoints, platforms, and systems to ensure that all activities and decisions to deliver safe quality services and outcomes
- Ensure effective governance and administration processes are in place supported by relevant policies and procedures
- Provide ongoing education, training, upskilling and coaching to the intake and assessment team
About You
We are looking for a motivated individual with experience in operationalising aged care legislation and program guidelines. In addition, the successful candidate will possess the following skills and attributes:
- A values-driven leadership approach and a passion for working in the non-profit space
- Ability to establish strong relationships with key internal and external stakeholders
- Exceptional influencing, negotiating and stakeholder management skills.
- Ability to operate in an ambiguous and changing environment where proactive and independent thinking and decision-making is required
- Strong analytical and evaluation skills including data analytics
- Exceptional communication skills with the ability to engage and influence stakeholders at all levels
- Previous assessment experience with CHSP and HACC Services (such as Transport, Domestic Assistance, Home Maintenance, Social Support, Meals) and NDIS will be highly regarded
