Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Holroyd and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing
The Pathways to Community Living Initiative (PCLI) is a key initiative funded under NSW Mental Health reform. PCLI is a coordinated, state-wide approach to supporting people with enduring and serious mental illness who have been in hospital for more than twelve months and or are likely to experience difficulty with discharge solutions, to re-establish their lives in the community.
 
The role of the PCLI Senior Mental Health Clinician is to provide care coordination for long stay adult patients transitioning to community based care, along with those at risk of long stay. This role requires contemporary skills and experience in a recovery-orientated, person-centred assessment and treatment model of care. You will play an integral part of transformational cultural change in supporting the shift away from institutional care to active citizenship in the community.
 
A primary goal of this position is to work collaboratively with staff, consumers, carers and other agencies (both within WSLHD and government and non-government organisations) to promote effective participation in Mental Health treatment, support and recovery. This role will carry a clinical caseload, alongside leading and championing practice improvement through participation in strategic and operational planning, quality improvement, service development, education and training, evaluation, research and other activities.

Essential Requirements: 

  • Qualification: Bachelor degree or higher level. 
  • Registration: Demonstrated full registration with AHPRA as an Occupational Therapist, Nurse or Psychologist, or eligibility to be a member of the AASW as a Social Worker (Registered Nurse requires at least 5 years full time equivalent post registration experience and approved post registration nursing qualifications relevant to the position)

Selection Criteria: 

  1. Demonstrated skills in Mental Health and discipline specific assessments and interventions and experience working with people with severe and persistent mental illness.
  2. Proven recent experience in a Mental Health leadership and/or change management role.
  3. Proven clinical experience within a multi-disciplinary team with the ability to provide consultancy, supervision, education and training on clinical issues.
  4. Demonstrated advanced written and verbal communication skills, computer competence and literacy, including the use of database systems.
  5. Proven organisational skills and ability to lead clinical staff and promote change in a positive manner, with excellent negotiation skills and the ability to deal with conflict.
  6. Demonstrated ability to collaborate meaningfully with consumers and carers with a commitment and understanding of recovery principles in Mental Health service provision.
  7. Demonstrated working knowledge of NSW Mental Health Act 2007.
  8. Current NSW Driver Licence.
